    Meet your Patient Intake Coordinator

    When a referral is submitted, a Patient Intake Coordinator (PIC) is assigned. They are responsible for ensuring all the necessary documentation is received. They will also work with the dialysis center to ensure your patient has a chair and a schedule that meets their needs. Using the admissions portal to submit a referral gives you direct access via chat to a PIC so you can ask any questions you have about the status of your patient’s placement.

    The portal will show you status updates so you know where the referral is in the process, and if you have questions or concerns about the referral, you can chat with your PIC within the portal. Any portal user within your facility will be able to see all referrals so there’s no disruption in service due to staffing schedules.
